Community Advisor helps senior living teams act on early warning signs before a resident ends up in the hospital.

It connects signals scattered across PointClickCare, explains why someone needs attention, shows the evidence, and turns it into a next step.

problem.

PointClickCare collects many signals, but these details aren't connected. How do we use this information in a meaningful way to create proactive care, instead of waiting for a resident's health to slip?

what I did.

• Pivoted from a value-based care dashboard to the Action Center. 6/6 customers called it high-value.
• Replaced "high risk" with "Confirmed". Customers could not tell high, medium and low risk apart. If it's there, it's important.
• Redesigned around evidence after the first AI model failed on real data.
• Required per-community settings for targets and thresholds.

impact.

• Beta: September 2026, on schedule, two months after we got the project in July
• General access: January 2027
• The Action Center is becoming its own product, and my card is reused by the alerts team.
